The aim of surgical treatment of osteochondrosis dissecans is to restore the blood supply to the necrotic piece of bone cartilage in order to prevent its detachment (articular mouse).
Retrograde O.D. drilling: The insufficient bone under the cartilage layer is drilled from retrograde (i.e. not through the cartilage layer) to allow a renewed blood supply.
Fixation of the dissecate: If the OD focus is already unstable and threatens to detach as a free joint body (joint mouse), the dissecat can be fixed using the arthroscopic technique with dissolvable pins or mini screws. Postoperative relief for 6 weeks.
Cancellous bone plasty/cancellous bone relining: If the bony part of the OD focus is very necrotic (dead), arthroscopic relining with the body's own cancellous bone and subsequent fixation of the dissecate can also be performed using the arthroscopic technique. Alternatively, OATS plasty can also be performed arthroscopically.
